The Types of Information We Collect

Personally Identifiable Health Information

When PII is combined with any information on your health or medical status, including billing information, we refer to this information as Personally Identifiable Health Information (PIHI). Specifically, we may ask for information on your personal and/or family medical history including height, weight, blood pressure, history of certain conditions and/or diseases and medication history. We may also ask you for information on your insurance plan and geographic location. We may ask you for PIHI, including your prescribing doctor’s name, any drug allergies and/or existing medical conditions, should you order prescription drugs or medical equipment through the Website.

We will use PIHI solely for the purposes for which it was provided, including for treatment and/or order processing and fulfillment. In accordance with HIPAA/HITECH, we will not otherwise use or disclose your PIHI without your express consent.

We may use a third party contractor to collect and/or store this information. In such instances, we will contractually ensure such third parties safeguard your PIHI in compliance with HIPAA/HITECH regulations.

If at any time you wish to have your PIHI removed, you may do so by submitting a written request to the notice address provided in the Terms & Conditions. Upon receiving a removal request we will remove such information from active databases and readily searchable media. Due to difficulty, cost and/or inaccessibility, information backed-up and stored separately and/or inactively will not be removed. In order to verify that it is the authorized user requesting removal of their personal information, we require you to send a signed statement, including your name, address, email address and birth date, in connection with the request to remove your PIHI.

Limitations

You agree that we are not responsible for (i) any disclosure of your personal information made by you to a third party through your use of the Site; or (ii) any disclosure of your personal information obtained illegally from us.

How We Use the Information We Collect
